Error during running Fuzzy Logic in Simulink

Dear all,

 I received an error about 'Input data dependency violation due to action subsystems'. I have no clue how to fix it because I believe my fuzzy logic controller is very simple and should not have any problem. I believe that it is due to my setting in Simulink or in FIS editor. Please help
